I knew 4 out of 5.  Missed the first one.  And since it's local(ish), I thought I'd share there is a Bethlehem Georgia.  Tons of folks go there to mail their Christmas cards.

Thanks, Donna.  Pennsylvania has Bethlehem and Texas has tiny town called Nazareth. (My brother & his wife are among the 350 residents.)
